System Nursing Governance Intake Form

Submit a request through System Nursing Governance (SNG). This intake form routes your request to the appropriate Core Council for review, prioritization, and action. You will receive updates on the request status via email.

    Request support developing nursing practice measures, reviewing performance trends, strengthening your ability to use quality, safety, or patient experience data, or scheduling a general data consultation.

    Request consultation for your evidence based project or idea.

  • Health Equity & Belonging Consultation

    Request consultation to address challenges in creating inclusive, fair, and equitable healthcare systems or workplaces.

  • Professional Development Consultation

    Request support related to professional development programs, recognition pathways, or Clinical Ladder advancement.

  • Nursing AI & Innovation Consultation

    Request review of an AI-enabled tool, automation, or technology initiative that impacts nursing practice.
